Steps to take after an accident at work

Follow these steps as soon as possible to get care and protect your rights.

Notify your employer immediately
Tell your supervisor or employer about what happened. They must complete Form CFSE-373: Employer's Accident Report.
Seek medical attention
If it's an emergency, go to the nearest emergency room.
Important: Keep a copy of your medical record.
You must then be evaluated by a doctor authorized by the CFSE.
Go to a CFSE office to file your case
Do it within 5 calendar days of the accident. You must submit:
  • Completed Form CFSE-373
  • Valid photo identification
  • Medical record (if you have already been treated)
  • Health insurance card (if applicable)

Complete the required forms
In the office, you will be guided and a file will be opened. You'll sign forms such as:
  • CFSE-1219: Consent for Treatment
  • CFSE-450.1: Affidavit if the employer failed to submit the form

Evaluate yourself with a CFSE doctor
A licensed doctor will examine you and determine the necessary treatment. You'll be able to receive medications, therapies and recommendations about your ability to work.
File the Employers' Report
The worker must report the accident to his supervisor or authorized personnel, and request that the employer complete the Form CFSE-373.
Options for completing the form:
  • Manually if the employer does not have access to the system
  • Print
Then, the injured person must come within five (5) days to a SIFC dispensary or to the Industrial Hospital.
If the employer refuses:
  • Go to a regional office or dispensary and report it in the Triage room.
  • In Filings you will be given a partially completed form and the CFSE-450.1 to try again with your employer.
  • If the refusal persists, provide a affidavit (CFSE-450.1) in the Filing Office.
Visit the Closest SIFC Facility
Show up at any dispensary, regional office or the Industrial Hospital.
In the area of Triage:
  • Nurses will take your vital signs
  • You'll be assigned a shift (not necessarily on a first-come, first-served basis)
  • If it's an emergency, you'll get immediate medical attention and then move on to the filing process

Do the Procedure at the Filing Office
At this stage:
  • Your physical and electronic file will be opened
  • Introduce your photo identification and health insurance card (if applicable)
  • Sign the CFSE-1219 (consent to treatment)
  • Deliver the Signed Employer's Report
  • If you live with Voluntary Report, you will be given a form for the employer
  • You will be given the Form CFSE-392, where you commit not to work if you are on a break
  • It generates your Graphotype card and you are assigned a random general practitioner
Get your Medical Evaluation
Once your case is filed:
  • Go back to the Immediate Care Room
  • The doctor will evaluate your condition, prescribe tests or medications if needed
  • It will decide if you should Take a break or go back to work
  • It will be completed on Form CFSE-395 with this determination
  • The nurse will review the orders before continuing the process.
Get your Medications (if applicable)
If the doctor issues a prescription:
  • This will be sent to the community pharmacy that best fits your needs
Schedule your Follow-up Appointment
After the initial medical care has ended:
  • Go to the area of Medical Appointments
  • They will inform you of the Date of your first appointment And the name of your general practitioner
  • That information will be noted in your Appointment card

Important Tips

  • Make sure that your employer does not complete the
  • Submit the Employers' Report at the nearest SIFC Filing Office
  • Bring: Photo ID, Private Health Insurance Card (if you have), Medical Record if you were treated at a private institution
  • Arrive early: the process includes several stages
  • If you are granted rest: The CFSE-395 It will indicate the duration and date of return, you must give a copy to your employer as soon as possible
